We are currently seeking a Home Health Registered Nurse (RN) with at least two years of nursing experience to join our dedicated team.

Position Overview:

As a Home Health Registered Nurse (RN) at Lux Home Health, you will be responsible for providing high-quality nursing care to patients in their homes. This includes conducting initial assessments, developing care plans, and working closely with physicians and other healthcare professionals to ensure the best possible outcomes for your patients. You will have the opportunity to form meaningful relationships with patients while playing a vital role in their recovery and ongoing health management.

Key Responsibilities:

As a Home Health Registered Nurse (RN), your responsibilities will include performing thorough initial assessments and developing individualized care plans that address each patient’s unique needs and health goals. You will provide wound care, and offer other treatments as prescribed by physicians. Additionally, you will collaborate with physicians, therapists, and family members to ensure a coordinated approach to patient care. You will also be responsible for maintaining accurate, up-to-date documentation of all patient care activities, while adhering to Lux Home Health’s internal policies and regulatory standards. Patient education is a key aspect of your role, as you will guide patients and their families on managing medical conditions, taking medications, and navigating their overall health. Finally, as a Home Health Registered Nurse (RN), you will respond promptly to any medical emergencies or patient concerns, delivering professional care and appropriate interventions when necessary.

Qualifications:

• Registered Nurse (RN) License in the state of Missouri.
• At least 2 years of home health nursing experience.
• Current CPR certification.
• Strong clinical skills and experience in providing skilled nursing care in a home setting.
• Ability to work independently, manage a patient caseload, and prioritize tasks effectively.
• Valid MO driver’s license, reliable transportation, and the ability to travel within the service area.
• Excellent communication, interpersonal, and organizational skills.
