Home Health Registered Nurse (RN) – Part-Time

Territory: Bellingham, MA, and surrounding communities

What You’ll Do

As a Home Health RN, you will manage an assigned group of patients and provide skilled, personalized care throughout their healthcare journey. Responsibilities include:

• Completing comprehensive nursing assessments

• Developing and updating individualized plans of care

• Performing admissions, routine visits, recertifications, and discharges

• Monitoring patient progress and identifying changes in condition

• Coordinating care with physicians, caregivers, and interdisciplinary team members

• Educating patients and families about medications, disease management, and self-care

• Helping patients remain safe and independent at home

• Providing proactive interventions that help prevent avoidable hospitalizations

• Completing accurate and timely clinical documentation

What You’ll Need

• Current, unrestricted Massachusetts Registered Nurse license

• At least one year of nursing experience in a clinical care setting

• Home health or home care experience is highly preferred

• Consistent availability for at least one full 8-hour day each week

• Valid driver’s license, reliable transportation, and current automobile insurance

• Ability and willingness to travel throughout the assigned territory

• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
