Job Details: Job Location: 201 - JOL Home Health Kyle - Kyle, TX, Salary Range: $34.50 - $34.50Hourly, The Licensed Vocational Nurse (LVN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). The LVN follows the plan of care established by the RN and physician, ensuring high-quality and compassionate care. This role involves direct patient care, documentation, and communication with the interdisciplinary team.

Essential Functions:

Provide skilled nursing care as outlined in the plan of care.
Monitor patient condition and report changes to RN and physician.
Administer medications and treatments as prescribed.
Educate patients and caregivers on disease management and care techniques.
Document all care provided accurately and timely.
Collaborate with interdisciplinary team to ensure continuity of care.
Maintain compliance with agency policies, procedures, and regulatory requirements.
Participate in in-service training and quality improvement activities. 
Promote infection control and safety practices during patient care.
Qualifications: Education:

Graduate from an accredited vocational nursing program.

Licensure:

Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. 
Current driver’s license.

Experience:

Minimum one year of clinical experience; home health experience preferred.

Skills:

Strong clinical skills, effective communication, and ability to work independently.

Environmental and Working Conditions:

Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases. 
Requires ability to travel locally and work a flexible schedule. 
May be exposed to unpleasant weather conditions.

Physical and Mental Effort:

Requires prolonged standing and walking, ability to lift up to 50 pounds, and move patients. 
Must be able to manage stressful situations calmly and courteously and make quick decisions to meet patient needs.
