Uplift HomeCare, LLC is seeking compassionate, dependable, and professional Certified Nursing Assistants (CNAs) to provide one-on-one care to seniors and veterans in the comfort of their homes throughout Polk County, Florida.

As a CNA with Uplift HomeCare, you will play an important role in helping patients maintain their dignity, independence, safety, and quality of life while remaining in their own homes.

Your responsibilities may include:

• Assisting patients with personal hygiene, bathing, dressing, and grooming
• Supporting activities of daily living (ADLs)
• Assisting with safe mobility, ambulation, and transfers, as appropriate
• Providing companionship and emotional support
• Assisting patients with maintaining a safe and comfortable home environment
• Supporting seniors and veterans in maintaining independence at home
• Observing and reporting changes in the patient's condition to the appropriate care team
• Following each patient's individualized plan of care and Uplift HomeCare policies and procedures
• Maintaining professional and respectful relationships with patients and their families
• Completing required documentation accurately and timely

About the Position

Uplift HomeCare provides in-home private duty care to seniors and veterans throughout Polk County, Florida.

Opportunities may be available in areas including:

Lakeland • Winter Haven • Haines City • Davenport • Auburndale • Lake Wales • Lake Alfred • Polk City • Dundee • Bartow • Mulberry and surrounding Polk County communities

Our CNAs provide individualized, one-on-one care designed to help patients remain safe, comfortable, and independent in their own homes.

Requirements

• Current Florida Certified Nursing Assistant (CNA) certification
• Current AHCA Level II Background Screening
• Valid Florida Driver's License
• Current automobile insurance
• Reliable vehicle
• Current CPR certification
• Alzheimer’s and Dementia training
• Minimum of 1 year of verifiable caregiving experience
• Ability to provide professional references and verify previous experience
• Compassionate, dependable, and professional approach to patient care
• Ability to follow an individualized plan of care and communicate effectively with the care team
• Reliable attendance and commitment to scheduled visits
