Responsibilities:
- Conduct in-home assessments and develop individualized care plans for clients.
- Administer medications and monitor patients’ vital signs and health status.
- Provide wound care, catheter care, and other nursing services as needed.
- Educate clients and families on health management and care techniques.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
- Document patient progress and maintain accurate health records.
- Respond to emergencies and provide immediate care as required.
- Build strong relationships with clients to enhance their comfort and trust.
Requirements:
- Current RN license in the state of Maryland.
- Minimum of 1 year of nursing experience, preferably in home health or community settings.
- Strong assessment, critical thinking, and communication skills.
- Ability to work independently and manage time effectively.
- Compassionate demeanor with a strong commitment to patient care.
- Basic Life Support (BLS) certification required.
- Reliable transportation and valid driver's license.
- Experience with electronic health records (EHR) preferred.
