Job Description
Metro is looking for a motivated and passionate part-time (with benefits!) Physical Therapist to provide in-home care. Responsibilities:
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
Qualifications
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients’ homes within the assigned geographic territory
Additional Information
The anticipated base salary range for this position is $65,000.00-$75,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.

The beautiful spa-like outpatient offices offer heated salt water pools, full fitness centers, and state of the art equipment including game ready ice compression, electric stimulation, cold laser and ultrasound heat. Our therapists spend 60+ minutes with every single patient and our patient outcomes are high because of how experienced our therapists are.
