Graceful Touch Home Care LLC is seeking a dependable and professional PRN Registered Nurse to conduct in-home assessments and follow-up visits for our clients.
Responsibilities
- Conduct in-home health assessments and evaluate client needs.
- Monitor vital signs and changes in health status.
- Develop and update individualized care plans.
- Complete required documentation accurately and promptly.
- Communicate concerns or changes to the agency.
- Provide follow-up and supervisory visits as assigned.
Position Details

- Visits typically take approximately 30 minutes

- Independent travel within assigned service areas required
Qualifications
- Current South Carolina RN license
- CPR/BLS certification
- Valid driver's license and reliable transportation
- Strong assessment, communication, and time-management skills
- Home care experience preferred
