Position Summary:

The Home Care Services Liaison is responsible for building and maintaining relationships with community referral sources.  This role serves as a key link between the agency and the community, ensuring awareness of services, supporting growth initiatives, and ensuring smooth transitions for patients onto home care and hospice services.

Key Responsibilities:

Build and maintain strong relationships with hospitals, medical practices, SNFs, ALFs and other referral sources
Educate referral sources on available home care and hospice services and admission criteria
Generate referrals through ongoing outreach, networking, and relationship management
Conduct regular in-person visits and follow-ups with referral sources
Collaborate with intake and home care/hospice leadership to ensure smooth patient transitions and admission to services
Represent the agency at community events, health fairs, and meetings
Stay informed on industry trends, regulatory changes, and competitor activity

QualificationsQualifications:

1-3 years of experience in healthcare marketing, home care, hospice or related field. 
Strong knowledge of home care and hospice services and referral processes
Excellent communication, customer services, and interpersonal skills
Self-motivated, goal-oriented, and able to work independently
High school diploma

Work Environment:

Primarily field-based with travel within service area
