Sage Group HealthCare Staffing is seeking Home Health PT for a 13-week travel/contract positions in Walla Walla, WA. We are accepting both local and travelers candidates.

Position Details:

Position Name: Physical Therapist (PT) Home Health

Location: Walla Walla, Washington

Employment Type:  Travel, Contract, Temporary

Contract Length: 13 Weeks

Pay Rate:  $3320 total weekly gross - $83/hour($49/hour W - 2 taxable , $34/hour non taxable stipend) 

Unit: Home Health

Shifts available: Days

Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm 

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri 

Position Requirements:

• Physical Therapist: 1-2 years - Required
• Recent/Current Home Health PT experience: 1 year within the last 3 years - Required
• Home Care Home Base (HCHB) experience - Required 
• OASIS experience - Preferred 
• Must have valid Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements:

• Degree in Physical Therapy - Required 
• PT License for State applying or Compact if applicable -  Required
• BLS Certification from AHA or American Red Cross - Required

Job Description:

• Provide Physical Therapy services for Home Health Patients
• Consult with the physician and an interdisciplinary team in the development of a plan of treatment
• Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
• Plans and prepares treatment plan based on evaluation of patient data
• Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
•  Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
• Drive to patient homes in the assigned territories on a daily basis.
• Complete charting and patient documentation in timely manner as per client policies and procedures
• Territories: Walla Walla County, lower part of Columbia Co and into Umatilla County 60 miles. They would be assigned to a geographic team and work with a manager and an interdisciplinary team
