As a Care Coordinator, you’ll help adults living with mental illness access the services, resources, and support they need. You’ll work closely with clients, families, healthcare providers, and community partners to coordinate care and ensure services are connected to each client’s goals and needs.
What You’ll Do
Build strong relationships with clients and develop individualized care plans.
Coordinate services and follow-up care across healthcare and community providers.
Connect clients with housing, benefits, financial resources, healthcare, and other community services.
Collaborate with families, providers, and treatment teams to ensure coordinated support.
Monitor client needs, progress, and changes in functioning, communicating concerns to the appropriate team members.
Provide supportive counseling, education, and assistance navigating services.
Support clients during times of crisis and help coordinate appropriate interventions.
Assist clients in developing skills that promote stability, independence, and successful community living.
Coordinate transitions and assist with discharge planning.
Maintain accurate and timely case notes, care plans, and required documentation.
Participate in team meetings, trainings, and community activities.
Provide coverage and support for team members as needed.
Follow agency policies and applicable regulations.
Perform other duties as assigned.
Qualifications: Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)
Minimum Required Education: Bachelor’s degree in social work, Psychology, Community/Family Counseling
Minimum Required Experience: Minimum of 1 years’ working with adults with severe and persistent mental illness.
Required License – Certification – Registration:
Issued By Governing Body (if applicable): Certification as a functional screener is not required but desired.

Required Auto Insurance: Vehicle liability insurance in accordance with Agency policy

• Experience with individuals with dual diagnoses is preferred.
• Experience with motivational interviewing is preferred.
Physical Requirements, Visual Acuity, and Work Conditions:
Physical Requirements: Work is performed largely in the community but may at times include an office environment or other various settings. This position frequently utilizes various office equipment including, but not limited to, a personal computer, multi-line telephone, fax machine, office copier. On occasion must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Must be able to safely push a person in a wheelchair on a level floor or on a wheelchair ramp in various weather conditions. Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift. Occasional use of stairs.
Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.
Working Conditions: The hours of work will generally be during regular business hours and normally work 40 hours a week. This position requires participation in reoccurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends. This position may work in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures.
