Summary
The Field RN will assess patient conditions, coordinate admissions, and develop ongoing care plans while providing emotional support to patients and families. They are responsible for maintaining accurate daily documentation and communicating significant findings to the Case Manager.
Job Description
Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.
Community Healthcare of Texas is currently recruiting for a Field RN.
POSITION SUMMARY
Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
ESSENTIAL FUNCTIONS
- Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
- Provide emotional support to patients and families while instructing them in activities of daily living.
- Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
- Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.
POSITION QUALIFICATIONS
- Registered Nurse in the State of Texas
- Hold a valid driver's license and have a dependable means of transportation.
- 2 years of nursing experience required
- 2+ years of experience in hospice strongly preferred
- Proficient skill using a computer is required; Microsoft Office Suite experience preferred.

Our Hospice Interdisciplinary Team of physicians, nurses, social workers, chaplains, and nurse aides work closely to provide the best plan of care to meet the needs of our patients and families.
Community Healthcare of Texas also provides perinatal and pediatric hospice and palliative care programs for those with a serious illness that are not ready hospice.
