PROGRAM/DEPARTMENT: DULCE

TITLE: Family Specialist

CLASSIFICATION: Part-Time (25 hours per week), non-exempt. 

SUMMARY: Family Specialists support health, wellness, and education for families in pediatric care. They build trusting relationships with families during well-child and acute care visits, and maintain contact through phone, text, email, or home visits. Using evidence-based education and nationally approved guidelines, they help families care for their children and follow care plans. They also connect families to resources for unmet economic, social, and legal needs. Their responsibilities include data collection, patient documentation in the EHR, and participating in quality improvement activities and training. This family-facing role reports to the program manager.

ESSENTIAL RESPONSIBILITIES, DUTIES, AND TASKS:

Responsible for establishing trusting relationships with patients and their families. 
Meet with families at routine Well Child Visits and telephone check-ins, supporting the parent-child relationship, providing information on child development, and supporting unmet social needs by coordinating referrals for concrete supports.
Provide ongoing parent/newborn support through phone, test, email, and community visits with a focus on parental and cultural strengths. 
Continually assess the need for additional resources for the family, especially relating to developmental concerns by building strong, honest, trusting partnerships with families that can address concerns in a supportive manner.
Collaborate with the legal partners to ensure families are provided concrete supports. 
Administer screening tools to ensure that families are supported in caring for their infants and adhering to the recommended plan of care.
Always maintain HIPAA and confidentiality compliance, strictly adhere to policies which ensure participant confidentiality.
Exhibit excellent working relationships with patients, visitors, colleagues, and facility personnel.
Engage in individual reflective supervision sessions.
Timely data entry into participant records and data collection systems. 
Prepare for and participate in weekly case reviews with the Interdisciplinary Team.
Participate in quality improvement activities.
Participate in outreach and community events.
Participate in related training and workshops. 
Perform other related duties as assigned or requested.

QualificationsREQUIREMENTS:

High School graduate or equivalent; associate degree or equivalent preferred.
Experience working in a community-based setting with young children (especially newborns) and their families, for at least 1 to 2 years preferred.  
Eligibility or certification as a Community Health Worker (CHW)
Working knowledge of Microsoft 365 applications such as Outlook, Word, Excel and SharePoint.
Be punctual for scheduled work and use time appropriately.
Commitment to improving infant and family health through innovative approaches that address the SDOH/HRSN.
Ability to work with many diverse people, to understand the community served and community connectiveness.
High energy, positive, “can do" attitude, flexibility, teamwork, and attention to detail: high degree of initiative.
Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; move materials up to 10 pounds on a regular basis; infrequently require moving materials weighing up to 20 pounds.  
Must be sensitive to the cultural and socioeconomic characteristics of the population that Champions for Children serves.
Bilingual, English/Spanish OR English/Creole, required.
Successfully complete a Level 2 criminal background check, motor vehicle records check and drug screening. 

COMPENSATION AND BENEFITS: 

PAY RATE - $24.46 per hour 

HOLIDAYS – Eligible Employees: PT/FT 25 hours and above
Champions grants 16 paid holidays each year :New Year's Day, Martin Luther King Day, Mindfulness Monday (3/13), Memorial Day, Juneteenth, July 4th, Labor Day, Veterans Day, Thanksgiving and the day following, Christmas Eve and Christmas Day, a Birthday Holiday, and 3 Floating Holidays (prorated for new employees).

PAID TIME OFF - Eligible Employees: PT/FT 25 hours and above
Champions offers Paid Time Off (PTO) to be used for vacation, illness, and personal time. New employees accrue PTO with each pay period at a rate equivalent to 15 days per year. PTO benefits increase on the 2nd, 5th, and 10th work anniversaries.
