Job Details: Job Location: Western Site - Chicago, IL, Salary Range: $110000.00 - $130000.00Salary/year,
Seeking a full-time Family Nurse Practitioner (NP) to provide primary healthcare to patients of Alivio. The Nurse Practitioner utilizes theories of psychosocial, developmental, educational and nursing sciences to facilitate the delivery of this health care. The Nurse Practitioner works inter-dependently with and independently of the multi-disciplinary team. Major functional areas will include: Practitioner, consultant, and educator.

Qualifications:
Graduate of an accredited Nursing School
Graduate of an accredited Post-Graduate Training Program
Board Certified or Board Eligible
Current IL State License, Controlled Substance License, and DEA registration.
Bilingual English/Spanish, required.
At least two years' work experience. Previous experience in a community health center or urgent care setting, desired.
