QUALIFICATIONS

Bachelor’s Degree required, with major course work in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, criminology, theology, nursing, counseling, education or other related social science or human services field.
Minimum of two years of post-secondary education or equivalent professional experience in a human services setting.
Minimum of two years of direct service or case management experience within a community-based, mental health, child welfare, or family support program.
Must have a valid driver’s license, acceptable driving record and access to reliable transportation.
Must have acceptable PA State Police, PA Child Abuse History and FBI clearances. 
Must be able to identify with the agency’s Mission Statement and support the agency’s Core Values. 

ESSENTIAL DUTIES OF THIS POSITION

 Family Service Delivery

Ability to adapt hours to needs of position.  May need to work evenings and/or weekends hours periodically including providing on-call emergency services on a rotating basis.
Ability to travel to and work independently in clients’ homes.
Focus on empowering family members to achieve greater independence through skill building, development of positive daily routines, strengthening coping skills, and supporting individualized goals that reduce reliance on professional systems and services.
Understand and implement the Family Empowerment Program’s developmental service model, including the three phases of intervention that require more intensive staff involvement during initial engagement and gradual tapering of services as families build skills, increase independence, and demonstrate the ability to meet their own needs.
Hold families accountable to program handbook requirements, expectations, and established program rules while supporting engagement and progress toward individualized goals.
Make day to day decisions regarding cases in the program and keep the Assistant Family Empowerment Director  informed of high-risk cases, pertinent issues related to clients and contracting agencies.
Engage in goal oriented, purposeful, intentional interactions with clients.
Provide on-call services as assigned and develop crisis plans for assigned consumers.
Implement evidence-based assessments and interventions as directed and support fidelity to Family Empowerment service models.
Maintain regular communication with the  Assistant Director/Director regarding program operations, case flow, and identified needs or challenges.
Provide direct services in alignment with Glade Run’s mission: Founded in Faith and Grounded in Excellence, We Empower Individuals to Experience Personal Wellness and Growth.
Model trauma-informed, culturally responsive, and strength-based practices in all direct work with families and collaboration with team members.
Participate in team meetings and contribute information related to service delivery, program trends, and family outcomes.

Community and Stakeholder Collaboration 

Serve as a liaison with county Children and Youth Services (CYS) to support referrals, admissions, transfers, and discharges in coordination with program leadership.
Participate in communication with courts, schools, and community partners as directed to support continuity of care and service coordination.
Attend community outreach and tabling events as requested to promote program awareness and strengthen referral relationships.
Support maintenance of referral pathways and stakeholder communication by sharing updates and relevant program information with leadership.

Risk and Safety Management 

Support timely reporting, tracking, and documentation of critical incidents and unusual occurrences in accordance with regulatory and agency policies.
Assist program leadership with follow-up actions related to incidents, including gathering documentation and participating in review processes as requested.
Maintain working knowledge of child and adult protective service laws and reporting requirements, including ChildLine, county protocols, and internal documentation procedures.
Serve as the “Manager in Charge” at various site locations to support implementation of emergency preparedness and continuity plans by following established procedures and communicating concerns to program leadership  

Service Documentation Coordination

Prepare weekly, monthly, and quarterly  progress reports for CYS.
Complete at designated chart audits for FEP programs 1x per month.
Assist with compiling outcome data, narratives, and documentation to support grant reporting, and funding requirements.

Knowledge, Skills and Abilities

Training in Family Empowerment models, Family Group Decision Making skills, Family Reunification, and Family preservation models. 
Working knowledge of maintaining a clean, safe, and health-supportive environment, including following infection control procedures and reporting safety or environmental concerns to program leadership.
Ability to adapt to changing priorities, respond appropriately to stressful situations, and support families and colleagues during challenging circumstances.
Demonstrates consistent attendance, dependability, accountability, and adherence to agency policies and expectations.
Applies sound judgment and initiative when addressing routine challenges and escalates concerns appropriately to program leadership.
Demonstrates the ability to prioritize multiple responsibilities, including direct service delivery, documentation requirements, scheduling coordination, and administrative tasks.
Demonstrates clear and professional verbal and written communication skills, including documentation, email correspondence, and participation in meetings with staff, families, and partners.
Demonstrated ability to use electronic medical records (EMR), Google Workspace (Docs, Sheets, Drive, Calendar), and other digital tools to support documentation, scheduling, and coordination of services.
Knowledge of how to access and coordinate community resources to support families’ needs.
Basic understanding of mental health diagnoses, behavioral health conditions, and common presenting concerns and ability to coordinate referrals when appropriate. 
Awareness of substance use and drug and alcohol issues and their impact on individuals and families and ability to coordinate referrals when appropriate. 
Working knowledge of child protective services law and mandated reporting responsibilities.
General understanding of the public welfare system and related supports impacting families served.
