Job Description
About the Role: Covenant Case Management Services is seeking compassionate and dedicated Direct Care Support Professionals to provide supported employment, respite, and community living support care for an individual with intellectual disabilities. You will play a crucial role in ensuring the individual's needs are met and providing a safe and supportive environment both in the home and the community.
Responsibilities:
- Provide one-on-one support, assistance, and companionship.
- Assist with daily living activities, including personal care, meal preparation, and medication administration.
- Ensure the individual's safety and well-being in the home and community.
- Maintain a positive and supportive atmosphere.
- Follow agency policies and procedures.
Qualifications:
- High school diploma or equivalent.
- Valid driver's license.
- Ability to pass a background check.
- Experience working with individuals with disabilities preferred but not required.
- Excellent communication and interpersonal skills.
- Compassionate and patient demeanor.

A dedicated case manager works alongside the individual and their support network to develop a personalized care plan, navigate Medicaid Managed Care, and ensure access to the right services at the right time.

Community Services: Community Networking, Supported Employment, and the Covenant DreamCenter Day Program
Residential Services: Alternative Family Living (AFL), Covenant AbleHaven Supported Living & CLS, and Covenant GracePause Respite Care
