What You'll Do:

Empower Daily Living: Guide and assist clients with daily living activities and personal care.

Build Community Connections: Support clients in joining local events, outings, and community activities.

Collaborate & Communicate: Partner with clients, families, and supervisors to tailor care to individual needs.

Ensure Client Safety: Maintain high safety standards and follow guidelines in both home and community settings.
What You Need:

High school diploma or equivalent (Required)
-
Valid North Carolina driver’s license and reliable transportation (Required)

Ability to pass a background check (Required)
-
1+ years of caregiving experience (Preferred)
Schedule:

Hours: Monday – Friday, 8:30 AM – 2:30 PM

Case management for IDD and TBI connects individuals to the services, resources, and care coordination they need to live safely and independently. A dedicated case manager works alongside the individual and their support network to develop a personalized care plan, navigate Medicaid Managed Care, and ensure access to the right services at the right time.
Our Services include:
Community Services: Community Networking, Supported Employment, and the Covenant DreamCenter Day Program
Residential Services: Alternative Family Living (AFL), Covenant AbleHaven Supported Living & CLS, and Covenant GracePause Respite Care
Clinical-Based Services: Covenant Thrive specialized therapy.
