JOB TITLE: Direct Support Professional 

CLASSIFICATION: Part Time, Non-Exempt

PROGRAM/DEPARTMENT:  Community Habilitation

IMMEDIATE SUPERVISOR:  Community Habilitation Manager

POSITION SUMMARY:  Responsible for in-home respite for disabled individuals and their families. Provides transportation, supervision, meal preparation, behavior management and participation in activities. Promotes, maintains and/or restores health, and/or minimizes the effects of illness and disabilities. Maintains accurate documentation of services provided including input in the program and/or needs of the people we serve. Responsible for acquiring and maintaining medication administration certification. Treats individuals and their correspondents with the utmost dignity and respect and allows individuals to fully exercise their human rights.

QUALIFICATIONS:

Minimum Education, Training & Experience:

Ability to perform all job functions

Preferred Education, Training & Experience:
High school diploma or equivalent

ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver’s license that meets the Agency’s driving policy
Work flexible hours, including weekends and holidays
Be a positive role model for people with disabilities and other staff

SALARY:
$17.50 per hour starting rate; $18 per hour after successful completion of medication certification
