Qualifications:

• Active state license
• DEA | State CDS
• Board certification required
• ACLS | BLS |PALS

Practice Environment:

• 153-bed Acute Care hospital 
• 4 ORs
• 2,842 annual cases
• Established, collegial ACT with 1 MD and 4 CRNAs
• Full complement of clinical services
