Duties
- Conduct comprehensive patient assessments to evaluate mental health status, substance use disorders, and physical health conditions using clinical judgment and physiological knowledge.
- Develop and implement individualized crisis intervention plans that prioritize safety, stabilization, and long-term recovery goals.
- Collaborate closely with hospital teams— and social work—to coordinate inpatient and outpatient care transitions.
- Manage electronic health records (EHR) efficiently while adhering to HIPAA regulations to ensure confidentiality and accurate documentation of patient interactions.
- Facilitate intake processes for diverse populations, including children, individuals with disabilities, and those from various cultural backgrounds, ensuring culturally competent care.
- Communicate effectively with patients’ families and support systems to foster understanding, compliance, and ongoing support for recovery efforts.
- This position will require in field work/traveling in the surrounding Houston areas.
Qualifications
- Demonstrated expertise in clinical case management within behavioral health or social work environments; experience working with individuals with mental health conditions or substance use disorders is essential.
- Experience working with children, individuals with disabilities, or diverse cultural backgrounds enhances your ability to deliver inclusive care.
- Strong task prioritization skills combined with excellent client communication abilities to manage multiple cases efficiently while maintaining high-quality patient service.
- Familiarity with electronic health records (EHR/EHR systems), HIPAA compliance standards, and medical records management is required.
