Be an individual 18 to 30 years old who has self-identified as a person who has first-hand experience with, emotional (mental health), behavioral challenges, and/or co-occurring disorders

As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing peer support, caregiver support and advocacy. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.

Responsibilities:

Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
Work with family members to identify goals and objectives of treatment
Provide services based on the needs as identified in the Treatment Plan
Coordinate with the child and caregiver on the scheduling and provision of services
Participate in relevant treatment meetings and attend all required trainings
Complete all documentation

Requirements and Education:

High School diploma or GED
Valid NYS Driver's License required
Be an individual 18 to 30 years old who has self-identified as a person who has first-hand experience with, emotional (mental health), behavioral challenges, and/or co-occurring disorders
Be able to use lived experience with a disability, mental illness, and involvement with juvenile justice, special education, substance use disorder, and/or foster care to assist in supporting youth in their resiliency/recovery and wellness
Must have, or are willing to complete the required state training for their Provisional Peer Credential
