Community Resource Coordinator

Job Details: Job Location: Chicago, IL 60623, Salary Range: $26.10 - $26.10 Hourly, The Community Resource Coordinator plays a key role in connecting individuals and families to services that promote stability, well-being, and self-sufficiency. This position coordinates community-based programming, provides case management and resource navigation, supports grant and program operations, supervises assigned staff and interns, and strengthens partnerships with community organizations.

Working closely with participants, Erie Neighborhood House staff, and external partners, the Community Resource Coordinator helps ensure high-quality, trauma-informed services while supporting program growth, compliance, and community engagement.

Participant Services

Provide comprehensive case  coordination, resource navigation, and referrals for participants and community members.
Support the completion of the enrollment process and identify the needs and connect individuals and families to appropriate internal and external resources.
Support walk-in participants and respond to referrals in a timely and compassionate manner.
Conduct home visits when appropriate to support participant engagement and service delivery.

Program Coordination

Coordinate, facilitate, and survey workshops, support groups, parent education sessions, and community presentations.
Collaborate with Adult & Family Programs staff to deliver high-quality, participant-centered programming.
Provide technical assistance and educational support to participants, staff, and community partners.

Data Management & Grant Compliance

Maintain accurate participant records and enter required information into agency databases.
Collect and organize program documentation, including attendance records, surveys, referrals, and other funder-required materials.
Monitor referral activity and support grant reporting, program evaluation, and performance tracking.
Ensure timely, accurate, and confidential documentation in accordance with agency and funder requirements.

Community Engagement

Build and maintain partnerships with community organizations, schools, healthcare providers, and other stakeholders.
Plan, coordinate, and participate in outreach events, recruitment efforts, and community collaborations.
Represent Erie Neighborhood House at community meetings, funder meetings, collaborative initiatives, and public events.

Other Responsibilities

Participate in agency meetings, trainings, and professional development opportunities.
Support continuous quality improvement initiatives across Adult & Family Programs and Community Wellness Support Services

Perform other duties as assigned to support the mission and goals of Erie Neighborhood House.

WORK ENVIRONMENT:

This position works in both office and community-based settings and requires regular interaction with participants, families, community partners, and Erie Neighborhood House staff.
Responsibilities include travel between Erie Neighborhood House locations, participant homes community organizations, and outreach events. Occasional evening and weekend hours are required to meet program and community needs.

Qualifications: Bachelor's degree in social work, Psychology, Child Development, Human Services, or a related field; or an equivalent combination of education and relevant professional experience.

Fluency in English and Spanish.
Experience providing direct services to individuals and families in community-based settings.
Experience with case management, resource navigation, and participant advocacy.
Strong organizational, communication, and time management skills.
Ability to manage multiple projects while maintaining accurate documentation.
Knowledge of trauma-informed and culturally responsive practices.
Commitment to social justice, equity, and serving diverse communities.
Successful completion of all required background checks.
Ability to work occasional evenings and weekends and travel throughout the community as needed.

Preferred Qualifications

Experience facilitating therapeutic groups and community workshops.
Familiarity with domestic violence, community violence, child abuse, crisis intervention, and case management.

PHYSICAL REQUIREMENTS

Sit, stand, and walk for extended periods.
Travel throughout the community and between Erie Neighborhood House locations.
Lift and carry materials weighing up to 20 pounds.
Operate standard office equipment, including computers and mobile devices.
Facilitate workshops, presentations, and community meetings.
