Position Summary

We are seeking a motivated, relationship-driven Community Outreach Coordinator to build referral networks and increase awareness of our non-medical home care services throughout the East Metro.

This field-based, part-time role (10–15 hours per week) is ideal for an outreach professional, former social worker, or healthcare sales representative who enjoys networking, building community connections, and helping families access dependable in-home care. You will represent CivicCare Solutions in the field, establish relationships with key healthcare referral sources, identify prospective clients, and coordinate qualified leads with our management team.

Primary Territory Focus

Primary Focus: Washington County (Woodbury, Stillwater, Lake Elmo, Oakdale, Cottage Grove, and Forest Lake).

Secondary Focus: Select East Metro & South Metro communities across Ramsey, Dakota, Hennepin, and Anoka counties.

Key Responsibilities

• Build Referral Networks: Establish and nurture relationships with hospital discharge planners, social workers, rehab facilities, senior living communities, care managers, elder law attorneys, and local senior centers.
• Field Outreach & Meetings: Conduct in-person visits, introductory calls, and professional presentations to introduce CivicCare Solutions’ private-pay home care services.
• Lead Generation & Follow-Up: Respond to initial care inquiries, gather preliminary client details, and coordinate consultations/assessments with management.
• Community Engagement: Represent CivicCare Solutions at local senior expos, health fairs, and networking events.
• Marketing Distribution: Distribute approved brochures, flyers, and promotional materials across key healthcare and community hubs.
• Tracking & Reporting: Maintain accurate records of outreach contacts, partner meetings, and referral activity using our tracking system, providing regular updates to leadership.

• Experience: At least 2 years of experience in community outreach, business development, healthcare sales, senior services, marketing, or social work.
• Communication Skills: Strong interpersonal, written, and verbal communication skills with confidence in making cold contacts and conducting in-person meetings.
• Availability: Flexible weekday business hours availability (when professional referral meetings occur).
• Self-Motivated: Strong organizational and time-management skills with the ability to work independently in the field.
• Tech Proficiency: Basic knowledge of Microsoft Word and Excel, and comfortable learning a simple web-based CRM/referral system.
• Transportation & Screening: Valid driver’s license, reliable personal vehicle, current auto insurance, a smartphone, and willingness to complete a standard background check.

Preferred Qualifications

• Established referral relationships with local healthcare providers, social workers, or senior care organizations in Washington County / East Metro.
• Prior sales or outreach experience in home care, senior living, hospice, or rehabilitation services.

• Pay Rate: $25.00 – $30.00 per hour (commensurate with experience)
• Bonus Structure: $250 client acquisition bonus for each qualified new client generated through documented outreach
• Travel: IRS mileage reimbursement for approved business travel
