About the Position

The Lakeland Hills Family YMCA is seeking a Temporary Community Integrated Health Coordinator to help launch and support community health and wellness initiatives that connect individuals to programs designed to improve health, reduce isolation, and strengthen their sense of belonging.

This position is ideal for an organized, outgoing professional who enjoys building relationships, connecting people with resources, and making a positive impact in the community. The Coordinator will work with healthcare providers, community organizations, YMCA staff, and participants to build referral relationships, support enrollment and participation, and track program activity and outcomes.

A key part of this role will be getting out into the community to meet with physicians' offices, healthcare organizations, and community partners. The Coordinator will introduce them to YMCA programs, establish referral connections, provide program information, and identify opportunities for future collaboration.

This is a hands-on opportunity for someone who is comfortable communicating with a variety of people, managing multiple priorities, following through on commitments, and representing the YMCA professionally throughout the community.

What You'll Do

Community Outreach & Partnerships

Build and maintain professional relationships with physicians, medical practices, healthcare organizations, care managers, public health agencies, senior-serving organizations, and other community partners.
Travel locally to physician offices and community locations to introduce YMCA programs, share information, establish referral connections, and follow up on partnership opportunities.
Serve as a professional point of contact for referral partners during the temporary assignment and ensure appropriate follow-up and transition of ongoing relationships to YMCA staff.
Educate community and healthcare partners about program eligibility, enrollment, schedules, participant support, and program outcomes.
Help coordinate presentations, health fairs, outreach events, site visits, and partner meetings.
Maintain organized records of partner contacts, outreach efforts, referrals, follow-up activities, and partnership opportunities.

Participant Support

Provide welcoming, compassionate, and professional service to participants, referred individuals, caregivers, and community members.
Assist with participant inquiries, screening, intake, enrollment, orientation, and follow-up.
Help participants navigate appropriate YMCA and community resources while maintaining professional boundaries.
Follow up with prospective and current participants to help reduce barriers to enrollment and encourage continued participation.
Create an inclusive environment where participants feel welcomed, supported, and connected to their community.
Assist with scheduling, attendance, participant communications, program materials, assessments, and special events.
Recognize when questions or concerns fall outside the scope of the position and appropriately connect individuals with the appropriate healthcare professional or YMCA leader.

Administration, Referrals & Data Tracking

Receive and process referrals accurately and within established timelines.
Maintain accurate records related to outreach, referrals, enrollment, attendance, participant progress, and program outcomes.
Track program participation, referrals, retention, and other key measures and assist with routine reporting.
Support participant feedback and program assessments as required.
Maintain confidentiality and appropriately handle participant and health-related information in accordance with YMCA policies and privacy practices.
Assist with documentation related to grants, accreditation, quality improvement, and reporting requirements.
Monitor program materials, forms, supplies, and outreach materials.

Communication & Teamwork

Communicate clearly, respectfully, and professionally with participants, healthcare professionals, community partners, and YMCA staff.
Work collaboratively with YMCA departments, including wellness, aquatics, membership, program staff, and organizational leadership.
Represent the YMCA positively and professionally throughout the community.
Participate in staff meetings, partner meetings, community meetings, training, and outreach activities as assigned.

Accountability & Professional Growth

Meet established goals related to outreach, referrals, participant follow-up, documentation, and reporting.
Identify opportunities to improve referral processes, participant access, partner communication, and program delivery.
Use feedback and program information to help strengthen the participant experience and community partnerships.
Maintain reliable follow-through and communicate challenges or delays to leadership.
Before the temporary assignment concludes, ensure partner contacts, referral information, outstanding follow-up, program records, and other key information are organized and transitioned to the appropriate YMCA staff.

Qualifications: Required Qualifications

Associate degree in public health, health education, healthcare administration, social services, exercise science, business, communications, or a related field. Equivalent relevant experience may be considered.
At least two years of experience in community outreach, healthcare relations, care coordination, health and wellness programming, customer service, or a related field.
Strong communication and relationship-building skills, with the ability to interact confidently with healthcare professionals, community partners, participants, and colleagues.
Excellent organizational skills and the ability to manage multiple contacts, referrals, deadlines, and follow-up activities.
Comfort using email, Microsoft Office or Google Workspace, databases, and basic reporting tools.
CPR/AED and First Aid certification, or the ability to obtain certification within the required timeframe.
Valid driver's license, reliable transportation, and the ability to travel locally to physician offices, community partner locations, and events.
Ability to work occasional evenings or weekends for meetings, outreach activities, or community events.

Preferred Qualifications

Bachelor's degree in health, human services, communications, business, or a related field.
Experience working with physicians, healthcare systems, referral networks, evidence-based health programs, chronic disease prevention or management, or community-based organizations.
Experience with participant data collection, outcome tracking, grant reporting, or quality improvement.
Knowledge of local healthcare and community resources.
Bilingual or multilingual communication skills.
