Summary
The counselor will implement and monitor valued outcome goals for individuals and participate in their training and supervision. They are also responsible for maintaining ongoing communication with the coordinator and utilizing community resources.
Job Description
You must have a valid NY State or out of state driver's license and a HS diploma/GED to be considered for this position. You must have your own vehicle for transporting the person supported when necessary.
The pay rate for this position is $19.00-$20.42 per hour, or higher depending on years of experience working in NYS OPWDD programs.

Some responsibilities include:

Participation in the training and supervision if the individual while incorporating the use of community resources..

Maintain on-going communication with the Community Habilitation Coordinator and attend scheduled meetings for discussions when necessary.

Implement and monitor valued outcome goal work with the Individuals.

Together with Geraldo Rivera, the reporter who first uncovered the state institution’s deplorable conditions, they established an organization dedicated to ensuring that those with developmental disabilities and autism are afforded opportunities to live life to the fullest.
At present we offer comprehensive services to over 1500 individuals and families in Queens, Nassau and Suffolk counties and provide supportive homes to over 250 people. Additional programs for both families and individuals include at home supports, skills development and behavioral intervention, social recreational programs and vocational training opportunities. We also offer collaborative school linkage programs with Long Island school districts supporting individuals and families in need of applied behavior analysis interventions.
Coordinated and focused through our unique Person-Centered Planning approach, all our services are provided with one goal in mind: To enable the individual to achieve their personal best, no matter what their abilities or challenges.
