Job Summary:
Performs basic client care and services in accordance with established policies and procedures and in accordance with plan of care established by licensed nurse. Demonstrates understanding of cognitive, physical and emotional needs of clients.
Duties and Responsibilities:
Provides personal care assistance to clients including bathing, dressing, grooming, eating, toileting and mobility.
Monitors skin condition and reports any change to the proper supervisory personnel.
Obtains and documents clients vital signs, temperature, blood pressure, respiration and pulse.
Prepares meals and monitors nutrition and fluid intake.
Provides medication reminders.
Assists in ambulation of clients including use of walkers, canes and wheelchairs.
Assists with client transfers using proper body mechanics.
Assists with household services including changing linens, laundry, light housekeeping, meal preparation and marketing.
Demonstrates understanding of infection control and promotes infection control standards by following universal precautions.
Reports changes in clients condition and any observed problems to the proper supervisory personnel.
Recognizes emergencies and can initiate appropriate response to the emergency.
Demonstrates sensitivity and responsiveness to clients comfort, privacy, needs and desires.
Maintains client confidentiality.
Reports immediately to proper supervisory personnel any known exposure to tuberculosis, hepatitis or COVID-19.
Documents client care and services on the designated forms.
QualificationsQualifications, Skills and Abilities:
High school diploma or equivalent required
Active & current state CNA Certification required
First Aid Certification
CPR Certification
Must be able to lift 50 pounds
Ability to read, write and follow oral and written instructions
