Certified Nursing Assistant (CNA) – Home Health

Prime Home Health is seeking compassionate, dependable Certified Nursing Assistants (CNAs) to provide exceptional in-home care to patients throughout the San Antonio area on a PRN basis. Help patients maintain their independence while delivering high-quality, one-on-one care in the comfort of their homes.
Responsibilities
- Assist with bathing, dressing, grooming, toileting, and mobility
- Monitor and document vital signs
- Provide medication reminders and assistance
- Prepare light meals and encourage proper nutrition
- Perform light housekeeping and errands
- Offer companionship and emotional support
- Document patient care accurately and timely in our EHR system
- Communicate changes in patient condition to the supervising nurse
Qualifications
- Current Texas CNA Certification
- Current CPR certification
- TB test taken within the past year
- High school diploma or equivalent
- Computer skills required (EHR experience preferred; Kinnser/WellSky, Slack a plus)
- 2 Years Home Health experience preferred
- Reliable transportation and valid driver's license
- Strong communication skills and a compassionate, patient-focused attitude
