Under the direction of the HCBS Supervisor or designee, the Clinical Float 1 provides flexible clinical and service coordination support across HCBS programs. The position completes time-sensitive assessments, visits, service planning, care coordination, incident follow-up, and temporary coverage based on operational needs, without maintaining a dedicated caseload. Work is performed under the oversight of appropriately licensed staff when required.

JOB RESPONSIBILITIES:

Primary Duties:

Provide temporary clinical and service coordination coverage across HCBS programs based on operational needs, without maintaining a dedicated caseload.
Conduct required in-person visits, assessments, reassessments, and significant-change assessments within established timeframes.
Develop and update person-centered service plans, disaster preparedness plans, and health and safety action plans, consulting appropriately licensed staff when required.
Coordinate waiver services, healthcare needs, provider communication, transportation, self-direction, and interdisciplinary care team activities.
Educate individuals and caregivers regarding health promotion, illness prevention, available services, community resources, and healthcare navigation.
Complete required follow-up and documentation related to critical and reportable incidents.
Monitor health, safety, service quality, member satisfaction, and continued program eligibility.
Provide temporary telephone, documentation, data-entry, and workflow coverage for Care Coordinators and Waiver Service Coordinators.
Support specialized and participant-directed HCBS programs, including enrollment, employer responsibilities, provider recruitment, financial management service processes, spending-plan development and monitoring, partner communication, required follow-up, and program documentation.
Maintain confidentiality and comply with applicable regulatory, contractual, Agency, and program requirements.
Qualifications: Required:

Registered Nurse in the State of Ohio or Licensed Social Worker in the State of Ohio; or
Licensed Practical Nurse in the State of Ohio or approved Social Work Trainee registration; or
Bachelor of Arts or Science degree in a field related to working with elderly and disabled populations and one year of experience per OAC 174-43-05 (B)(2)(d).; And
At least one year of experience working with persons with disabilities/chronic conditions and LTSS

Preferred:

Previous Care Management experience
One year of experience in home and community-based care, geriatrics, behavioral health, or long-term care. 

Knowledge of chronic disease management
