Summary
The Client Services Navigator provides universal intakes, assessments, and case management services to eligible clients. They coordinate with internal departments to facilitate referrals and assist with program applications.
Job Description
The Client Services Navigator-Community Services provides universal intakes, assessments, referral to TVCCA programs, application assistance for DSS programs, and information and referral services to clients in need of general case management services. Client Services Navigators work with the department’s Energy Assistance, Financial Literacy, and SNAP Outreach staff to provide comprehensive services to TVCCA’s eligible client base. They are responsible for following up on all referrals as well as providing ongoing, outcome-oriented case management. This position must utilize TVCCA’s Tribeware case management software and become knowledgeable of the Results Oriented Management Accountability (ROMA) as well as the Human Services Infrastructure service model. TVCCA operates under these two management principals and all employees must understand and adopt them.

Housing: Assisting households facing eviction or homelessness. Housing Services can also help with the search and application process for senior housing options in New London County.
Energy Assistance: Financial assistance toward a household’s energy bill; emergency assistance if a home’s service is shut-off or about to be, or out of fuel; other energy related services including heating system repair and replacement, budget counseling, and referrals for weatherization.
Nutrition Assistance: WIC is a supplemental food/nutrition education program for eligible pregnant, postpartum, and breastfeeding women, and infants/children to age 5. TVCCA’s Meals on Wheels delivers hot, nutritious meals to seniors, allowing them to remain independent in their home.
Case Management: Accessing programs within TVCCA, the State of Connecticut Department of Social Services, and throughout the community like food banks, furniture banks, and housing applications. TVCCA also helps to prepare new DSS applicants for eligibility determination interviews.
Financial Education: Supportive services designed to foster and develop positive household budgeting and credit management skills, and to assist households with overcoming their current hardships by taking command of their financial futures.
Childcare and Preschool Programs (Early Head Start, Head Start, and Little Learners): TVCCA provides affordable, quality, comprehensive infant/toddler, pre-school & childcare programs that serve children from birth through age five.
