Job Details: Job Location: Mental Health Services - La Verne, CA, Salary Range: $27.01 - $33.02Hourly, SUMMARY:

Under limited supervision, works independently in the familys home and a variety of community and/or treatment settings, coordinates services with an understanding of the complexity of each family while blending the critical area of the youth with the facilitation to support and enhance the efforts of youth and their families/caregivers, system workers who are part of the support team and other individuals in a variety of environments. The incumbent in this position will provide facilitation, assessment and case management services including all related documentation for services planned and provided.

JOB DUTIES:

Responsible for delivery of therapeutic interventions/services as established by the Client Care Coordination Plan and the Needs and Service Plan. Interventions include individual, family or group format. All interventions/services will be consistent with established standards of practice and standards of proficiency discussed and established in clinical and administrative supervision
Verifies the client meets medical necessity by processing the Clinical Assessment and developing a Client Care Coordination Plan for delivering services to meet the clients needs. Clinical assessment and Client Care Coordination Plan will meet standards for clinical competence and best practices, as reviewed in supervision and quality assurance oversight
Collaborates effectively with internal and external departments and agencies, DCFS, Probation, family/foster members to assist with successful integration and delivery of services, and transition into the community. Coordinates and works cooperatively with all designated Haynes staff to ensure a team approach to delivery of treatment for each child and family
Monitors and documents the individuals progress toward meeting the personal objectives outlined in the Client Care Coordination Plan. Ensures required documentation of case consultations, individual and family therapeutic interventions to track progress and increased level of functioning. Completes all documentation in accordance with deadlines and performance standards established by the Mental Health Policy and Procedures Manual and/or the Director of Mental Health
Provides Evidenced-based Practice (EBPs) interventions to clients commiserate with the appropriate training, education, and respective model adherence requirements, including but not limited to, booster courses, conference calls, dashboard guides, EBP-specific supervision, respective certification requirements and in consideration of the individualized needs of each client
Ensures compliance with and timeliness of documentation, productivity standards and service delivery. Documentation is to be turned in according to established schedules
Initiates, updates and coordinates the initial, revised, and annual Client Care Coordination Plan, Assessment and any other required Department of Mental Health documentation needed to verify medical necessity and deliver services.
Maintains knowledge of Title 22 and all regulations, policies, and procedures pertaining to services with children receiving out-of-home care
Attends all scheduled clinical and quality improvement in-services, company-based training, and treatment team meetings to develop, review, and implement quality improvement service delivery practices and to develop and maintain effective clinical skills
Meets with the Clinical Supervisor or designee, weekly to review job performance, access support and guidance in working with severely emotionally disturbed children and their families, and identify areas needing improvement with action plans for performance enhancement or training. These areas may include additional trainings and encouragement for further professional growth and development to improve job performance
Obtains necessary documents, such as previous psychological and social history records; reviews and maintains all records with timeliness, accuracy, legibility, and clinical appropriateness. Prepares and forwards all reports required by State, County, and other agencies, on a timely basis
Performs other reasonable related duties as assigned

PHYSICAL REQUIREMENTS:

In the course of performing this job, the incumbent typically spends time standing, walking on sloped/hilly areas of campus, carrying, driving, lifting (up to 40 pounds), listening, speaking, reaching, writing, and participating in restraint proceduresQualifications: QUALIFICATIONS:

Bachelors Degree (BSBMA) in Psychology, Social Work, Counseling or related field Preferred
Experience serving individuals with complex needs including Severely Emotionally Disturbed (SED) youth/families and/or youth residential settings
Experience with the Mental Health, Child Welfare and/or Juvenile Probation Systems Preferred
Preferred MHRS and licensure eligibility
Bilingual in Spanish both written and verbal preferred
Proof of valid California drivers license and personal vehicle insurance required. Must maintain a good driving record and be an acceptable driver to the companys insurance provider
Must fulfill all requirements of employment, including clearance of all required background records (e.g. Department of Justice, Child Abuse, California Community Care Licensing, County Sanction List, etc.), physical examination, and drug testing
