Position Summary: The Child and Adolescent In Home Therapist - Family Functional Therapist plays a key role within the Children and Family Treatment and Support Services (CFTSS) framework by providing intensive, evidence-based family therapy to youth ages 10–18 and their families. The clinician delivers services primarily in the home and community using the structured FFT model to strengthen family relationships, improve communication, reduce behavioral and emotional concerns, and promote long-term stability. Responsibilities include comprehensive assessment, treatment planning, weekly family and/or individual therapy, care coordination, crisis support, and ongoing documentation. The selected candidate will receive specialized training, supervision, and ongoing support in the FFT model to ensure effective and fidelity-based service delivery. This position is hybrid and allows up to two (2) remote workdays per week following successful completion of onboarding and required training. Remote work is a discretionary privilege based on program needs, performance, productivity, and compliance with agency expectations and may be modified or rescinded at any time.

Essential Accountabilities

Provide evidence-based clinical services to children, adolescents, and families utilizing the Functional Family Therapy (FFT) model. The selected candidate will receive specialized training and ongoing support in the FFT model, primarily serving youth ages 10–18 and their families.

Maintain a caseload of approximately 10–15 families, providing services for an average duration of 8–26 weeks, consistent with FFT model requirements and program expectations.

Provide a minimum of weekly, one-hour family therapy sessions, primarily in the home and community, with scheduling flexibility, including evening and other nontraditional hours, to accommodate family availability and support treatment engagement.

Conduct comprehensive clinical assessments, identify behavioral health needs, and develop appropriate diagnoses and treatment recommendations within scope of practice.

Develop, implement, review, and maintain individualized treatment plans consistent with identified needs, family goals, and program requirements.

Provide individual, family, and/or group clinical interventions, as clinically indicated and consistent with program requirements.

Deliver services using a strength-based, family-driven, trauma-informed, and person-centered approach that promotes engagement, stabilization, skill development, and improved family functioning.

Provide crisis intervention and clinical support to assigned clients and families, in person or by telephone, when available and clinically appropriate.

Complete timely and accurate documentation of all clinical services, including progress notes, collateral contacts, assessments, treatment plans, telephone contacts, and other required program documentation within established deadlines.

Coordinate care and maintain appropriate communication with Care Managers/Care Coordinators, schools, behavioral health providers, and other involved service providers, as applicable.

Adhere to FFT fidelity standards, including required clinical practices, data collection, outcome monitoring, referral requirements, reporting expectations, and model-specific documentation.

Complete all annual and as assigned required state, agency, program and FFT trainings, individual and/or group supervision, consultation, fidelity monitoring, and other professional development activities.

Complete required Mandated Reporter Training and recognize and report suspected or known child abuse or maltreatment in accordance with applicable law and agency policy.

Adhere to all applicable confidentiality laws, regulations, agency policies, and professional standards.

Promptly report identified or suspected youth high-risk behaviors, safety concerns, significant clinical concerns, and reportable incidents in accordance with agency protocols and applicable reporting requirements.

Maintain required Direct Service Units and other established productivity expectations.

Submit a weekly work schedule to the Supervisor and report changes to the schedule as they occur.

Utilize Paycom to accurately record all hours worked in accordance with agency policies and procedures.

Accurately track and submit mileage reimbursement documentation within established agency timelines.

Participate in Division-wide trainings, program meetings, staff events, workshops, conferences, OMH trainings, and other required professional development activities.

Maintain reliable access to a personal vehicle throughout the workday and transport youth and/or family members when clinically appropriate and required for service delivery.

Maintain required automobile insurance coverage and submit updated proof of insurance prior to expiration.

Adhere to all requirements and protocols established within the agency Safety Policy and other applicable health and safety procedures.

Perform all other duties and responsibilities as assigned to support effective program operations and service delivery.

QualificationsQualifications: The preferred candidate will be a Licensed Practitioner of the Healing Arts (LCSW, LMSW, LMHC, LMFT) with at least one year experience working with children and families. Must have a clean valid New York State Driver's License and reliable personal vehicle.
