Job Summary:

St. Elizabeth Healthcare is hiring a Certified Nurse Assistant for our Home Hospice unit.

Shift/Hours: PRN, 1st (8:00AM-4:30PM)

Job Description:

The Certified Nurse Assistant Home Hospice provides direct and indirect patient care under the supervision and direction of the RN case manager.  Assists patients with activities of daily living (ADL), provides for personal care, comfort and assists in the maintenance of a safe and clean environment for an assigned group of patients.

• Patient Care Ability to provide all ADL’s for Hospice patients

• Communication Reports pertinent information such as changes in client condition or behavior to the case manager or Assistant Nurse Manager.  Understands and has ability to follow the C.N.A. plan of care as directed by the case manager.

• Interdisciplinary Team Participation Attends IDT meetings when required and communicates significant information as it relates to personal care.

• Documentation Documents completely and accurately on itinerary and visit records. Documentation turned into the office within 24 hours of visit.

• Personal Growth & Development Attends and seeks out in-services to maintain certification.  Has knowledge of regulatory requirements as related to position.

• Performs other duties as assigned.

Education, Credentials, Licenses:

• High school or GED graduate.

• Completion of state-approved Nurse Aid 75-hour course. Not listed on the State Abuse Registry where applicable. Ability to read and write effectively in English.  Successful completion of Nurse Assistant exam required for certification. 

OR

• Nursing Assistant (Nursing Student) – Successful completion of first clinical rotation in an accredited nursing program.  Active student in Nursing Program.

Specialized Knowledge: 

• Demonstrates basic mathematical skills.  Basic life support.

• Computer literate.

Kind and Length of Experience: 

• Successful work employment history.

Desirable

• Prior experience as Nurse Assistant in hospital/home care/long-term care.
