Job Summary:
Responsible for the case management process including assessment/reassessment, service planning, and monitoring of Morton patients.
Essential Duties and Responsibilities:
- Identifies, assesses, and works with high-risk patients
- Conducts initial screenings to determine eligibility
- Develops comprehensive treatment plans
- Communicates with care team
- Engages with patient to assure understanding of goals and assists in meeting goals, monitoring progress
- Coordinates delivery of service
- Leads Transition of Care (TOC) activities
- Refers all identified social drivers of health to Community Health Worker or designee
- Coordinates with interdisciplinary team and documents all services provided
Requirements:
- Current OK nursing (LPN or RN) license in good standing is required.
- 2+ years of demonstrated clinical experience in healthcare.
- 1+ years of demonstrated Clinical Case Management or Care Management experience is required.
- Bilingual in Spanish preferred, but not required
- Current BLS/CPR certification required.
- Demonstrated ability to work effectively with staff and the public.
