Summary
The Case Manager is responsible for managing patient care, determining medical necessity using InterQual criteria, and facilitating timely discharge planning. They must collaborate effectively with the healthcare team to ensure positive patient outcomes in an acute care setting.
Job Description
Regularly exposed to the risk of bloodborne/airborne disease; contact with patients under a wide variety of circumstances; exposed to unpleasant elements (accidents, injuries, chemicals and illness); subject to varying and unpredictable situations; able to handle emergency, crisis or pressured situations; subject to multiple interruptions; occasionally subject to extended or irregular hours; requires judgment/actions that impact patient outcomes.
Graduate of a nursing program. Bachelor's Degree is preferred.
Three (3) years of clinical nursing experience in an acute care setting is strongly preferred. Case management and/or Utilization Review experience also preferred. Requires good communication skills and the ability to work collaboratively with all patient populations and members of the healthcare team. Must be able to determine InterQual criteria for medical necessity, understand timely discharge planning and authorizations.
Current Mississippi RN licensure is required. BLS certification is required
