What You’ll Do
Track and record patient case information, progress, and evaluate recovery.
Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance.
Work with team members to ensure discharge planning goals and objectives are developed.
Schedule appointments, and coordinate care
What You’ll Need:
Excellent verbal and written communication with professional phone etiquette.
Minimum of 1 year Case Management experience preferred
Customer service oriented
High attention to detail
High School diploma or GED
Must be able to pass a pre-employment background check and drug screen
