Case Manager

Job Details: Job Location: Oklahoma Recovery Center - Oklahoma City, OK, Salary Range: $20.00 - $25.00Hourly, Job Shift: Day, Current Oklahoma Case Management Level I (CM I) certification required.

Experience in substance use disorder treatment, behavioral health, or residential treatment preferred.

Knowledge of addiction, recovery principles, relapse prevention, and community resources.

Ability to coordinate individualized case management services and identify barriers to treatment.

Experience with discharge planning, referrals, and coordination of care preferred.

Ability to maintain accurate, timely, and compliant documentation in accordance with HIPAA and applicable regulatory standards.

Strong organizational, interpersonal, and communication skills.

Ability to collaborate effectively with a multidisciplinary treatment team.

Must successfully complete a background check and maintain professional and ethical standards.

Primary Responsibilities

Provide case management services to clients receiving residential substance use disorder treatment.

Assess client needs related to housing, transportation, employment, healthcare, and community support.

Coordinate referrals and link clients to appropriate community resources.

Assist with discharge planning, aftercare coordination, and continuity of care.

Collaborate with therapists, medical providers, and other treatment team members.

Maintain timely and accurate case management documentation.

Advocate for clients and assist them in overcoming barriers to recovery.
