Are you an experienced caregiver who enjoys helping others, solving problems, and being someone your team can count on?

Freedom Home Care is looking for a Caregiver Lead who can provide exceptional one-on-one client care while also supporting and mentoring fellow caregivers.

This is a hands-on caregiving position with additional leadership responsibilities. Our Caregiver Leads play an important role in helping new caregivers feel supported, maintaining high standards of care, and helping our team respond when client or scheduling needs arise.

What You'll Do

As a Caregiver Lead, you'll continue providing direct care to clients while also serving as a resource and role model for other caregivers.

Responsibilities may include:

• Providing one-on-one care in clients' homes
• Assisting with personal care, mobility, meals, medication reminders, housekeeping, companionship, and other care-plan needs
• Mentoring and supporting new and current caregivers
• Helping caregivers understand client routines, expectations, and care plans
• Assisting with caregiver introductions and client transitions
• Helping cover open shifts and urgent scheduling needs when available
• Responding to caregiver and client needs during assigned on-call periods
• Communicating concerns and changes in client condition to the appropriate office or nursing team
• Completing accurate and timely care documentation
• Modeling professionalism, reliability, communication, and appropriate caregiver boundaries
• Working closely with our office, scheduling, and nursing teams to support quality client care

Schedule & On-Call Expectations

This position requires flexibility and availability to support both our clients and caregiving team.

• Monday–Friday
• One weekday on-call rotation: 8:00 AM–10:00 PM
• Rotating weekend on-call: 8:00 AM–10:00 PM 
• Must be willing and able to travel between Mankato, Owatonna, and surrounding client locations
• Schedule may vary based on client and staffing needs

We'll discuss the specific schedule and on-call rotation during the interview process.

Qualifications

Applicants must:

• Have at least 1 year of healthcare or caregiving experience
• Have a valid driver's license
• Have their own reliable personal vehicle
• Maintain current auto insurance
• Meet Freedom Home Care's driving record requirements
• Be willing and able to travel to Mankato, Owatonna, and surrounding client locations
• Successfully complete a Minnesota DHS background study
• Meet required TB screening requirements
• Be comfortable providing personal care
• Be able to meet the physical requirements of assigned client care
• Be available for the required weekday and rotating weekend on-call schedule

Previous leadership, mentoring, CNA, or home care experience is a plus.
