Care Transition Coordinator - Remote

The Care Transition Coordinator reports to the Manager of Care Coordination and supports the Care Transitions team in navigating patients between Gentiva care settings. This role is responsible for referral coordination, chart review, and ensuring patients are placed in the right care setting at the right time. This is a telephonic, work-from-home position operating under established performance criteria.

The schedule is Mon-Fri 4pm-12am. 

As a Care Transition Coordinator, You Will:

• Review medical records against nationally recognized clinical criteria guidelines to determine eligibility for care levels

• Document and track patients in Gentiva systems and send referrals as directed by the Director of Care Transitions

• Focus on placing patients in the right care setting at the right time

• Collaborate closely with Care Transitions Nurses to ensure smooth patient transitions

• Provide education regarding Home Health, Hospice, and Palliative Care services

• Assist with the admissions process by coordinating with Gentiva branch staff to follow up on patient admission status

• Maintain awareness of department performance objectives and assure patient satisfaction goals are met

• Participate in special projects and perform other duties as assigned

• Adhere to and participate in mandatory Company training, including HIPAA, Business Ethics, Compliance, and other policies and procedures

• Uphold Company policies and procedures while promoting core values

About You

Qualifications – What You’ll Bring:

• Call center experience in a healthcare setting

• Ability to quickly learn and master information regarding locations and services

• Excellent computer skills and familiarity with multiple software applications

• Strong verbal and interpersonal communication skills with empathy for patients and families

• Excellent customer service and analytical problem-solving abilities

• Good time management and ability to prioritize workload

• Associate degree preferred

• Minimum of 1 year of healthcare or healthcare call center experience

• Reliable high-speed internet connection (minimum 10 Mb/s download and 2 Mb/s upload; satellite internet not acceptable)

• Knowledge of Home Health, Hospice, and Palliative Care services

Preferred Qualifications (Not Required):

• Previous experience in a Care Transitions or Patient Navigation role

• Familiarity with electronic medical records (EMR) systems

• Prior experience with post-acute care coordination

• Additional certifications in patient navigation, care coordination, or healthcare management
