The Care Manager is responsible for providing quality professional care management services to all clients and their responsible parties. Our objective is to assist our clients in managing and navigating challenges in aging as well as Adults with physical and mental disabilities and providing the highest quality of life. This includes:
- Care Coordination
- Managing home health aides
- Medical oversight
- Interfacing with medical personnel
- Advocacy, information and referrals

Full Service Professional Care Management and In Home Care
Aging Life Care Managers provide solutions for older adults, individuals with chronic illnesses or special needs, and their families.
Care Managers are credentialed professionals with degrees in Social Work, Gerontology, Nursing or Human Services. Our Certified Care Managers (CMC) have earned a national certification recognized after meeting the education, experience requirements and passing the national exam. Care Managers have extensive knowledge of costs, quality, government entitlements, eligibility, community resources and availability of services for older adults.
In Home Care:
Elder Options, Inc. employs only the best in-home care providers.
Prior to being hired, we require:
Complete Federal background investigation, application with education, employment, certifications, and personal references, DMV report, auto insurance, negative TB test, face to face interview, orientation ; mentor training, CPR training is offered to all employees. Regular paid in service training is provided by Care Managers and CEU units are provided.

Conduct assessments to identify problems, eligibility, and need in client's home, Develop a plan of care, monitor services, Identify and assist with financial, legal, and/or health issues, etc.
Home Care Includes:
Assistance with personal care including lifting and transferring when needed, Meal planning, preparation and clean up, Light housework, Medication reminders and reorders, Transportation, etc.
