Who We Are

Vigilance Health partners with FQHCs, ACOs, IPAs, and payers to deliver Population Health and Care Management services that actually move the needle. We run Care Management, APCM, Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs — and we measure success the way our partners do: quality metrics, patient engagement, and outcomes that show up in data.

The Role

The Care Manager is the heart of Vigilance. You'll lead the charge in building meaningful relationships with patients — driving better health outcomes and empowering people to take control of their own healthcare. You'll build trust, rapport, and accountability using the principles of Motivational Interviewing and clinical best practices, call after call, patient after patient.

What You'll Own

Patient Outreach & Engagement

You'll drive a high volume of outbound calls every day — ramping from roughly 45 calls and 5–20 patient connections a day in your first month to 75+ calls and 22+ connections by month three — because consistent outreach is what makes everything else possible.

Relationship Building & Rapport

You'll build genuine rapport with your assigned patient panel using Motivational Interviewing techniques, turning first conversations into ongoing relationships patients trust and stay engaged with.

Risk Stratification & Panel Management

You'll learn the risk stratification of your assigned panel inside and out, grow your “billable” and “at risk” patient panels, and work to minimize the “fallen out” panel — keeping patients engaged before they disengage.

Care Coordination & Goal Setting

You'll set PCP visit smart goals with appropriate patients and drive tangible improvements in health outcomes — PCP visits, medication adherence, care navigation, and more — through consistent, engaged care management.

Documentation & Clinical Best Practices

You'll translate every conversation into clear action and solid documentation, applying clinical best practices so the story of each patient's progress is captured accurately and can be acted on by the rest of the care team.

What Good Looks Like in 90 Days

• You're consistently making 75+ calls a day and connecting with 22+ patients daily.
• You've built real rapport across your assigned patient panel and understand their risk stratification cold.
• Your “billable” and “at risk” patient panels are growing, and your “fallen out” panel is shrinking.
• PCP visit smart goals are set with the right patients, and your care management conversations are producing genuine Success Stories.
• You're driving tangible improvements in health outcomes — PCP visits, medication adherence, and care navigation.

What You Bring

Experience That Matters Most

• LVN/LPN license is required
• A track record of connecting with people quickly — you can build trust with a stranger in the first few minutes of a call
• Comfort with a high-volume, fast-paced outbound calling environment
• Familiarity with Motivational Interviewing techniques, or the ability to pick up structured conversation frameworks quickly
• Strong active listening skills — you can translate a conversation into clear documentation and a concrete next step
• An organized, self-directed, proactive problem-solving mentality
• Experience in healthcare, care management, or a patient- or customer-facing outreach role is a plus

Nice to Have

• Experience with medication adherence, chronic disease management, or PCP visit navigation
• Prior use of a care management platform or EHR
• Bilingual or multilingual communication skills highly preferred
