At Chronic Care Staffing (CCS), our remote LPN/LVN Care Coordinators use their nursing experience every day to support patients managing chronic conditions. You’ll build ongoing patient relationships, recognize changes in health, reinforce care plans, and coordinate with healthcare teams—all from your home office. Monday–Friday. No weekends. 100% remote. Still making a difference.

What Your Day Looks Like

You’ll manage an assigned patient population and spend much of your day connecting with patients by phone. You’ll:

• Review symptoms, medications, chronic conditions, and care needs 

• Make proactive outbound patient calls 

• Recognize changes in patient status and escalate concerns appropriately 

• Reinforce patient education and care plans within your scope of practice 

• Coordinate with providers, pharmacies, and other members of the healthcare team 

• Document patient interactions accurately and efficiently in the EHR 

• Manage patient follow-ups and ongoing care needs 

• Consistently meet quality and productivity expectations, including at least 2 billable patient encounters per hour

This is an active, phone-focused nursing role. You’ll need strong clinical judgment, great communication skills, and the ability to efficiently manage multiple patients and systems throughout the day.

What You’ll Need:

• Unencumbered active LPN license. Compact/multistate HIGHLY preferred.

• Recent nursing experience in primary care, specialty care, chronic disease management, or a high-volume outpatient setting 

• Strong clinical judgment and patient communication skills 

• Strong EHR documentation and computer skills 

• Ability to independently manage a high-volume patient workload 

• Dedicated, private workspace appropriate for HIPAA-compliant patient interaction

• Active BLS certification, or willingness to obtain prior to start date. 

Experience with CCM, RPM, TCM, population health, telephone triage, or remote nursing is a plus.
