Overview:

Sentara Health Plans Roanoke is looking to hire a Care Coordinator- LPN.

Sentara Health Plans is hiring a Remote Care Coordinator for the Roanoke/ Lynchburg/ Danville area!

This position is remote work from home with travel to Nursing facilities required.

Hours of operation are M-F from 8:00am to 5:00 pm  

Applicants must be in for the Roanoke/ Lynchburg/ Danville area.

The Care Coordinator performs a variety of casework duties and provides case management services to Members in Nursing Facilities, including patients, families, and designated caregivers and requires travel to assigned facilities.   Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Education:

• Associate's degree in Nursing (preferred).
• Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.
• Licensed Practical Nurse or Bachelor's prepared health professional strongly (preferred).

Certification:

• LPN strongly (preferred)

Experience:

• MCO/HMO experience
• 3 years of clinical experience within the last 5 years.
• 3 years of Medicaid/Medicare experience (preferred).
• Familiar with HEDIS measure (preferred)
• Healthcare (preferred)
• Case Management/Care Coordination experience (preferred)
• Experience with Community Health Resources (preferred)
• Experience initiating care plans and both subjectively and objectively conducting assessments (preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $29.60 - $43.54 hourly.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
