Summary
The Patient Care Coordinator will visit adult day care centers to connect seniors with behavioral health services and build relationships with families and staff. They will also provide interpretation services and ensure continuity of care while maintaining accurate documentation.

Compensation
$23–$25 per hour

Mindify Wellness and Care is seeking a compassionate, outgoing, and bilingual Patient Care Coordinator to help older adults access psychiatric, therapy, and behavioral health services throughout Northern and Central New Jersey.
This role is ideal for someone who enjoys building relationships, educating others, and making a real difference in the lives of seniors and their families.
You'll work directly within our partner adult day care centers, helping participants understand available services, connecting individuals to care, and serving as a trusted resource within the community.
What You'll Do
- Visit partner adult day care centers throughout New Jersey
- Build relationships with participants, families, and staff
- Educate individuals about available behavioral health services
- Help connect patients with psychiatric and therapy services
- Support patient engagement and continuity of care
- Serve as a liaison between patients, families, providers, and center staff
- Provide Spanish-English interpretation when needed
- Maintain accurate documentation and uphold HIPAA standards
What We're Looking For
Required
- Fluent in English and Spanish
- Excellent active listening skills, with the ability to build relationships and work effectively within different adult day center environments, policies, and procedures.
- Comfortable speaking with individuals and groups
- Valid driver's license and reliable transportation
- Willingness to travel throughout New Jersey
- Organized, dependable, and self-motivated

Schedule
- Monday–Friday
- Primarily daytime hours
- Typically between 8:30 AM – 2:30 PM
