Job Details: Job Location: Bristol Hospice - Salt Lake, UT - SALT LAKE CITY, UT, Position Type: FT Hourly, Salary Range: $30.00 - $33.00Hourly, Job Shift: Day, The Bereavement Coordinator is responsible in planning, implementing and maintaining a bereavement program to meet the needs of families/caregivers from Bristol Hospice for up to one (1) year following the death of the individual hospice patient

Qualifications, Duties, Perks: An Average Day: 
(Includes, but not limited to)

Coordinate the assessment and delivery of grief counseling needs and services (one- on-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers
Plan for Bereavement Program development, expansion and refinement through annual evaluation of services
Provide bereavement support to hospice personnel coping with work related grief through one-on-one counseling, supervision of interns providing one-on-one counseling, and referral to community resources
Provide bereavement information and referral services to callers from the community
Expand and maintain community resource list, as needed
Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups)
Function independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community
Participate in hospice activities such as in-service education, hospice personnel meetings and relevant committees
Advance professional knowledge and skills
Other duties as assigned

Requirements: 

Must possess a bachelor’s degree or equivalent specialized experience or theological degree from a college or university – OR – master’s degree in social work with qualifications of experienced clinical grief counseling
Must have experience with families/caregivers and patients through death/dying issues
