Location: Brooklyn Court (Schermerhorn location) 

Days/Hours: Wed – Sunday, 12PM – 8PM (with rotating on call schedule)

Fortune has grown steadily over the years to an agency with over $90 million in annual budget with approximately 600 staff. We anticipate that we will keep growing, both in size and depth of service. Fortune has a dual mission: both advocacy and service. The advocacy portion of our mission focuses on building a more just criminal legal system and reducing the barriers to reentry into society. The service side of Fortune has evolved to encompass a broad range of programs that started with employment and education and now includes a robust array of services, among them: alternatives to incarceration, supervised release, court advocacy, discharge planning within correctional facilities, licensed substance use and mental health treatment, a housing continuum ranging from emergency and transitional through permanent congregate and scattered site housing, assistance in obtaining benefits, a care management unit, HIV services, a food and nutrition program, and an arts program. Each service we provide is informed by the needs of the population we serve. Fortune currently serves approximately 18,000 people a year. Our program models are recognized, both nationally and internationally, for their quality and innovation. We have locations in Brooklyn, Bronx, Manhattan, and Queens. 

Position Summary: 

The Fortune Society, in partnership with Community Healthcare Network (CHN), will provide court and community-based, person-centered, voluntary support for individuals with unmet needs who are released back to the community following arraignment at Brooklyn Criminal Court on Schermerhorn, or from other Brooklyn court parts as requested by court stakeholders. This program will operate from Wednesday – Sunday, 11:00 AM – 10:00 PM to accommodate both the heaviest court day parts and the after-hours periods when there are fewer resources available in the Brooklyn court area. The program’s structure will require in-court collaboration between Fortune’s court advocates, peer navigators, and court stakeholders, as well as a direct connection to the clinical professionals on the mobile medical unit (MMU), which will be operated by CHN. The Field Supervisor will be responsible for the seamless onsite management of this initiative. This position will also require being on call when not in the field to handle staff emergencies or general inquiries.

Time Management: 

This person will demonstrate excellent time management skills including making, keeping and/or rescheduling appointments on their own personal calendar as well as psychiatric appointments for clients on their caseload.  The successful candidate will be excellent at maintaining personal organization in pursuing numerous tasks in parallel, managing his/her/their time to maximize efficiency, and will be good at completing paperwork accurately and in a timely manner.   All of this requires good computer navigation skills as well as effective typing speed.   

Supervisory Skills: 

The successful candidate will possess strong interpersonal skills and communicate effectively.  S/he will also have excellent networking and operational skills, with the ability to analyze problems to identify effective solutions and pivot priorities at a moment’s notice.  This person will be able to foster a positive work environment, providing constructive feedback where necessary, motivating staff, and supporting professional development. The candidate will also oversee a team of clinical staff, which includes weekly individual supervision.  This will include supporting staff in their professional development, completing 6-month and annual evaluations to ensure quality professional measures are met, addressing performance and discipline issues when necessary.   

Preference will be given to candidates with a strong working knowledge of New York State Health Home Care Management policies and procedures. 

Duties and Responsibilities:

Supervise a team of care managers and peer navigators; Provide programmatic supervision and guidance to the MMU project staff to ensure that all project outcomes and deliverables are achieved and address any challenges or difficulties that arise; oversee a team of clinical staff, which includes weekly individual supervision.
Support supervised staff in their professional development which includes performing weekly supervisions, completing 6-month and annual evaluations to ensure quality professional measures are met, and addressing performance and discipline issues when necessary
Ensure services are delivered in alignment with program policies and Fortune’s mission;
Ensure all daily operations are appropriately managed, reviewing staff schedules for adequate coverage, managing schedule changes and personally providing coverage for care managers or peer advocates in the event of an unexpected call out;
Provide on-call support to staff during the mobile unit’s hours of operation; this will be done on a rotational basis with CHN’s project coordinator
Ensure accurate data collection on key performance metrics for all services and the proper use of required reporting tools;
Collaborate with data analysts and internal quality assurance staff on accurate data reporting;
Serve as the main point of contact for CHN and key stakeholders within the Brooklyn courts;
Assist in ensuring the effective implementation of the Evidence-based practices (EBP), including Screening, Brief Intervention, and Referral to Treatment (SBIRT) and Motivational Interviewing (MI), as essential components of the MMU program;
Play a leadership role in building and strengthening working relationships between partner
organizations, including CHN, to maximize effectiveness of community infrastructure for providing services;
Attend and participate in all necessary MMU meetings, conferences, and trainings;
Advocate for needed services, support with resources and internal or external referrals;
Maintain ongoing communication with supervisor;
Perform other duties as assigned.

 
Qualifications: Qualifications: 

Bachelor’s degree or higher preferred, but will consider applicants with relevant experience and appropriate skills; 

Two or more (2+) years preferred of program management and supervisory experience; 

Experience with criminal legal system & population, housing, case management, substance use and addictive behaviors, and mental health a plus; 

Ability to work independently with off-site supervision; 

Ability to work efficiently and effectively in a deadline-driven environment, juggling multiple priorities; 

Bilingual in Spanish a plus;

Excellent documentation and time management skills; 

Strong Microsoft Office and database entry skills; 

Able to work with people from diverse backgrounds in a culturally competent manner;
