BEHAVIORAL HEALTH CONSULTANT - LCSW or PSYCHOLOGIST

POSITION SUMMARY:

This position is responsible for integrated behavioral health care. The Behavioral Health Consultant will be part of an integrated primary care team. Scope of work will include but not limited to screening, assessing, triaging and managing patients in a primary care setting. Position requires availability to provide services during regular hours of operation.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provides care to adults, children, and adolescents utilizing a biopsychosocial-spiritual-cultural assessment and treatment approach.
Provides consultation, screening, and follow up services to patients as part of an integrated primary care team.
Formulates behavioral health interventions appropriate to a primary care setting and assists with implementation and monitoring of shared treatment plans with the primary care provider.
Assists primary health care providers in recognizing and treating mental disorders and psychosocial problems.
Works as a team with medical providers to coordinate care for patients. 
Assesses the clinical status of patients referred by primary care providers through brief consultative contacts.
Works with primary care team to triage patients and develop a shared treatment plan which may include initiating referrals to specialty behavioral health care as appropriate.
Assists in facilitating coordination of care with the care coordination staff and/or other designated care team staff.
Assists in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.  Analyzes population data to identify high leverage interventions within patient panels.
Assists in preventing relapse or morbidity in conditions that tend to recur over time.
Teaches patients, families, and staff care, prevention, and treatment enhancement techniques.
Provides targeted assessments and evaluations, including diagnostic impressions and the utilization of screening tools.
Maintains familiarity with verbal de-escalation techniques and acts as resource to staff.
Consistent attendance and punctuality is required and expected to work the hours set by ANHC and/or the schedule set by supervisor. 
Effectively utilizes electronic health record (EHR) to document patient activity and manage patient population.
Expected to complete BLS/CPR and any other required trainings deemed necessary for this position.

Serves as a member of site committees as requested.
Attends and participates in meetings and activities as required.
Provide trauma informed and trauma specific treatment.

PHYSICAL, SENSORY, ENVIRONMENTAL QUALIFICATIONS:

The work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. 

While performing the duties of this job, the employee is regularly required to use normal range of hearing and eyesight to assess patients as well as to record, prepare and communicate in writing, in person and by phone.  Occasional bending, stooping, lifting, and stretching.  Requires eye-hand coordination and manual dexterity sufficient to operate a computer, telephone, and medical equipment.   An individual in this position may be exposed to communicable diseases and other diseases common to health settings, blood-borne pathogens, and physical hazards.

Work is performed in a healthcare clinic and office setting.  ANHC is a tobacco-free campus.

Qualifications

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 

Work Experience: Must possess specialized work experience which involves conducting behavioral health assessments, formulating diagnosis or treatment recommendations and providing therapy services to patients. Zero to three years in practice since licensure. Experience in a primary care or safety-net clinic setting preferred.

Education, Certification and Licensure: Master’s degree from an accredited graduate school of social work or related field from a regionally accredited university. Maintain a current, active license with the State of Alaska as a Licensed Clinical Social Worker. Must maintain current BLS/CPR certification.

Additional Skills & Knowledge: Bilingual preferred.
