Alternate Nursing Administrator

AOM Infusion is looking for an Alternate Nursing Administrator – Per Diem to join our team. This is a per diem opportunity located in Naples, FL.

POSITION OVERVIEW

Responsible for overseeing the operations and management of the Florida Home Health Agency in the absence of the Regional Nurse Manager. Responsible for the clinical management and scheduling of services for assigned groups of patients. Responsible for providing/performing patient related nursing services.

KEY RESPONSIBILITIES 

• Coordinate all patient care services for an assigned group of patients with other members of the health care team.
• Coordinate patients start of care with referral source/discharge planner in the absence of the Regional Nurse Manager.
• Obtain initial orders/prescription for care. Complete the plan of treatment at the start of care and update as care needs change or every six months.
• Complete initial telephonic assessment with the patient to determine medical needs, functional limitations and willingness to participate in care.
• Maintain accurate and complete information in Specialty Pharmacy Business Operating System) for assigned groups including, but not limited to: demographic information, medical information (height and weight, diabetes status, past medical history), nursing care provider, contact persons.
• The Alternate Nursing Administrator will assure that the patient’s plan of care is implemented as written and updates as the plan changes.
• The Alternate Nursing Administrator will assure that patient needs are reassessed by a licensed health care professional when there is a significant change in the patient’s condition, when requested by the patient’s physician and after discharge from the hospital.
• Provide clinical support to the patient, field staff and pharmacy staff.
• Based on the initial telephonic assessment and in conjunction with pharmacy, determine the appropriate administration method for prescribed therapy.
• Documents pertinent medical information into the Specialty Pharmacy Business Operating System including but not limited to the medication profile. 
• Participate in case conferences as needed to discuss pending referrals, current patients and clinical issues.
• Performs skilled nursing visits to all ages of patients.

• The Alternate Nursing Administrator must be available to the agency at all times either in person or by telecommunication in the absence of the Regional Nurse Manager/Administrator.
• Any other responsibility as designated by the employee’s supervisor.

REQUIREMENTS

• Graduate of an accredited nursing program.
• A registered professional nurse licensed and currently registered in the state of Florida and in good standing with the Board of Nursing.
• Level 2 background check and clearance through AHCA.
• 2 years of experience in home care or home infusion therapy.
• Knowledge of ACHC standards and accreditation process.
• Knowledge of regulatory requirements for home care in Florida.
• Good communication skills, detail-oriented and organized.
• Good computer skills
