About the Role:

As a Social Services Assistant at Columbine West Health and Rehab, you will provide services to residents and families who are faced with social, emotional and medical stressors. Serves as a resident rights advocate in all manner of speaking.

Join our team as a Rehab Unit Support Specialist! In this role, you will:

• Assist with admissions and discharges, advocating for residents and their families.

• Collaborate with Social Workers, facility departments, and community resources.

• Help residents exercise and protect their rights, creating individualized care plans.

• Provide resources for discharge and post-discharge planning.

• Ensure timely and accurate documentation and assessments.

• Coordinate and manage ancillary services, resident council, and neighborhood meetings.

• Schedule and participate in Care Conferences and interdisciplinary meetings.

• Support staff in managing social and emotional factors, crisis intervention, and audits.

• Adhere to facility policies, including HIPAA requirements, and act as a mandated reporter.

• Perform additional administrative and social services duties as needed.

• Be a part of a compassionate team dedicated to enhancing the lives of our residents! All other duties as assigned.

What We're Looking For:

Experience and Education:

• An associate’s degree in human services preferred.

• Experience in healthcare preferred.

• Experience working with the geriatric population preferred.

Skills:

• Ability to effectively able to read and interpret documents.

• Ability to communicate effectively.

• Basic mathematical skills.

• Ability to reason and problem solve.

• Basic technical skills.

Our Total Rewards package includes a salary range of between $17 - $19/hr. DOE
